This document outlines the terms and conditions of assessment and treatment provision from Brain & Mind Ltd and its associates.

Appointments

Face to face and remote appointment costs are the same
Unless otherwise specified, our face to face appointments take place at our consulting rooms at the Platinum Medical Centre, 15-17 Lodge Road, London, NW8 7JA

Cancellation policy

The full price of any scheduled sessions (including time set aside for travel (if applicable) will be charged for cancellation of any appointment within 48 hours of the scheduled appointment.
If the patient is not self-funded, all cancellations fees are payable by the solicitors or agents acting on behalf of the referred client.

Remittance of invoices is requested within 28 days. Failure to pay within 28 days may lead to further action being taken and your details being passed on to debt collection where necessary.

Report & clinic letters

Clinic letters and reports will be shared with the funder and/or the patient’s general practitioner/approved medical practitioner, as necessary to ensure the clinician has appropriately and safely transferred the duty of care for the patient’s psychological well-being. The patient will also receive a copy of this letter/report unless otherwise specified during the appointment.

Onward referral letters, assessment, progress, and discharge reports will be produced as a matter of clinical procedure and are not optional.

If there are specific deadlines that need to be kept, please identify these as soon as possible.

Records & patient data

We would also ask that all relevant medical documents, for the purpose of the assessment, be made available as soon as possible. An appointment date cannot be confirmed until we are in receipt of the documents.

We only collect data that is relevant and necessary for your treatment / management in order for us to deliver the best possible service. We process the data because it is in our legitimate interests as expert clinicians to do so.

We do not collect any information which does not have a specific purpose or role to play in your care.

Please note: We are unable to provide an emergency or out of hours service. In the event of a crisis or medical emergency you should contact your/the patient’s GP, case manager or care coordinator in hours and your local emergency service or A&E out of hours. If you or the patient is having a mental health crisis you can also contact your local crisis line.

We would be grateful if you could confirm your acceptance of these terms and conditions by signing and dating below. Until your detailed referral and acceptance have been received in writing, we will not be able to confirm an appointment date and time.
